Populate phone number "country code" from selected National Society on data collector registration form

1. Split the phone number field on the data collector registration form into Country code and Local number
2. Populate the country code based on selected national society field.
3. Cascading works both ways. That means, when a user change a country code, the National Society field also changes.

I heard some rumors that we want to support multiple phone numbers for each data collector, correct? Then I assume that they will all have the same country code?
