Field Report Enhancements

Background

There are a number of styling changes to the field report that were specified throughout the development of #1074.

This issue splits out concerns addressing #1004 so that essential functionality can move into production and allow extra time to finesse the look of the field reports.

Features:

• [x] Title is automatically populated
• [x] Minor form validation issues: fields accepting numbers do not check if number is an integer
• [x] Reset the province field when the country is changed.
• [ ] Update ‘yes/no’ questions to be able to submitas empty (will automatically choose ‘no’ if none is selected since these fields are booleans)
• [x] Add tool tip or other feature to explain abbreviations
• [ ] Add upload options

  Since uploading documents is a new feature the dev team need to know that the expected functionality is that documents will be uploaded if available and they need to go to the emergency page as attachments under “Response documents”.

• [x] Update colors:
• [ ] Fix titles and texts, also is need to add some letters (the blue ones in the screenshot).

• [ ] Change the guidance in the event of an EPI field report. If possible, it should read, “ Please edit this title by replacing “Epidemic” with the name of the specific disease” The dev team needs to take note of this feature .

A lot of things in this ticket are still very valid clean-ups and improvements we should do. Am moving this to 4.3, and we can discuss either making separate tickets for some specific items in here that should still be addressed and closing this, or clarifying this a bit to include things that still need to be done.
