Publish MHDS in IG publisher #166

This publication is already converted into markdown. Seems this should be published in html. https://github.com/IHE/IT-Infrastructure/tree/master/MHDS Note this does not include conversion to IG publication, but could be done with IG publisher.

Should NOT change content, so that the end result is simply a new publication without public comment phase. Follow the pattern used in HIE-Whitepaper for style and layout.
