MHD Improvements #175

There are many reported issues with MHD as multiple projects implement. This work item would look to resolve the current backlog of issues and provide a revision. Note that some of the issues reported likely will result in significant change to MHD and thus MHDS. such as changing ITI-65 to an $operation, and support for Bundles as documents.
