the canonical uri is fixed in the instances.fsh, and not on the right path

current

I think should be

url = "http://profiles.ihe.net/ITI/PDQm/CapabilityStatement/IHE.PDQm.client"

or might be

url = "http://profiles.ihe.net/ITI/PDQm/CapabilityStatement/capabilitystatement-IHE.PDQm.client"

also fix server.
