Updated capability statements and ITI-90 to simplify the required search parameters.

I would propose to keep the following parameters on Organization:

_revInclude=OrganizationAffiliation:participating-organization _revInclude=OrganizationAffiliation:primary-organization

These allow a client working with Organizations to easily get the associated OrganizationAffiliation resources.
