This PR is meant for discussion.

Added in this pass:

  1. Add section on simple use cases that don't require mcsd. This was just a brainstorm and may happily be removed.
  2. Add explainer on actors, transactions, resources.
  3. Add clarification on CHWs, volunteers, unlicensed roles. The latest mcsd spec is not imperative in how to capture these roles.
