Medication: add medication details #32

I suggest to ad to the model Detailed medication information, so that we can cover the case when more details about medication are needed beside those already listed in the model.

The datatype might be of type Class and the implementation could be realized extending the current details element as Reference to Definitional Medication resource or MedicationKnowledge

DataModel was updated. I did not add an extension for a reference to MedicationDefinition in the profile. It felt out of proportion for now. However, I created a Jira ticket to add reference options to Medication.definition data element in R6. https://jira.hl7.org/browse/FHIR-46414
