Reimplement upsampling simulation #62

We found that high-resolution absorption coefficients together with a low resolution fluence yield surprisingly accurate initial pressure distributions that behave much better for the k-wave simulations.

This feature was lost during the refactoring frenzy and it would be nice to get this feature back.

Upsampling module with list of input fields and upsampling strategies (and probably axes to upscale). Add a module that combines mua and phi to p0.

@kdreher I do not remember, did we want to include this into 1.0.0 still? I think pushing it back might be better for now?

I agree, I think we should revisit this after 1.0.0
