Add a notice that this is a beta version #72

Beta to be "the version" for a few months in case someone notices a cardinal issue etc.

Resolved by adding a notice into the preambles on all pages where users fill in the form: " NOTE: If you notice a bug, or have a suggestion for the improvemenet of this form, we'd be grateful if you posted an issue on GitHub: https://github.com/INCF/artem-is/"
