Port Largo's disclaimer widgets into this plugin #5

Changes

plugin version number is now 0.1.0
copies Largo's widget into the plugin
replaces Largo optionsframework and metabox APIs with standard WordPress ones
keeps Largo widget name, widget CSS class, post meta options

Why

For #3 and #2.

Gonna merge this, then add the INN/largo#1500 docs to this plugin once that's done.
