The facilitator can create up to 3 custom questions in Step 6 of survey setup. However:

In the popup window, change "Label" to "Question"
It's not clear where the user would customize the response options for each question they create. For instance, if they select "Select multiple", then where can Facilitator identify what those multiple options are?
Clicking "Save question" leads to a Server 500 error.

@svasdev Should we allow only Open Ended(text) and Integer as in the mockup

Quick Fix will be to allow simple types:

integer
decimal
text

And later allow complex types:

select_one
select_multiple
date
time
dateTime
image
file

@svasdev Will that address the current survey needs?
