In 6.4.18.12, the second formal definition should be deleted or changed. #268

Justification: The second formal definition is like following:

numDerivativesAtStart:Integer[0..1]=0; numDerivativesInterior:Integer[0..1]=0; numDerivativesAtEnd:Integer[0..1]=0;

, which are already defined in previous paragraphs and this paragraph, so it can be deleted.

Or, If they need to be, then "numDerivativesAtStart" should be changed to "numDerivativesStart" and "numDerivativesAtEnd" should be changed to "numDerivativesEnd". Proposal: edit accordingly.
