Add support for http4s 0.22 #229

http4s 0.21 and 0.22 are binary-incompatible and some of us use http4s-0.22 (i.e. because we want newest features but still CE 2). PR is quite simple cause it just copies everything from http4s-0.21 module and fixes imports/deprecations.
