Excerpt from icinga-user mailing list:

Under the API docs (http://docs.icinga.org/latest/en/pluginapi.html), it states that a return code of 1 for host state can mean both UP or Down/Unreachable, and then has an asterisk next to it, presumably meaning clarification is available elsewhere. Unfortunately I am not finding that clarification anywhere on that doc.
