Inconsistent questions about entry date

Continuous residence affidavit - using users[i].last_entry_date_month and users[i].last_entry_date_year (from screener) should be users[i]..date_of_last_entry (from id: U.S. entry info and travel documents) - this is what shows on I-821.

I think we should:

• Use just one question for this
• Make it ask for the entry date using 3 parts, with the day being optional

I think this was somewhat inconsistent feedback about whether we always need to ask for the day or just use month/year (because applicant won't know the exact day). but we should add a new 3-part question using a grid for each component as described above, and logic to show the day when it's defined only
