Remove/Update the following:

Remove KIM de-identifier folder along with history
Remove API mapping with history and reintroduce it after removing any treatment centre related references
Reintroduce templates if they contain references to any of the centre/trail names
Reintroduce the example Jupyter notebooks (in both this and the client libraries repo) after removing any centre related information, including in the output fields
