ATX-style Markdown #4

Sub item from Issue #3

All headlines should use ATX-style Markdown (that is, 1 or more #-characters to lead in the headline), and they should match the original HTML.

Also, all headlines should be separated from previous and following paragraphs by (at least) one empty line. (While this is not strictly needed for Pandoc to correctly process the Markdown, it makes for easier reading (and possibly, automatic/scripted checking/patching/modifications).
