Support primary/secondary DICOM server #186

Similar to OHIF, it is very beneficial to be able to fetch images from one server, and annotations from another. I believe we discussed this, and I even thought there is an issue about it, but I could not find it.

As discussed, this is about configuring URL - not UI - and handling just one more server is sufficient. For this initial work we do not have a need to support n servers.
