Rename patients #23

This is a big set of commits that does the following:

renames directories to be in line with our new friendly names
adds patient resources that match our friendly names
updates IDs for existing patient objects to match friendly names
updates Readme.md with new friendly name

This pull request addresses issue #22. Once we update the DICOM metadata .txt files we should be able to close that issue.
