Moving the DICOM files form Amazon S3 into GitHub #50

You'll see that I've moved the FHIR stuff into a dedicated directory, then added a top-level directory for the DICOM data. I've also update the readme file to reflect these changes and also fix a few styling issues with headers.

Cancelling pull request as we've agreed to re-arrange the repository by using sub-modules.
