Update FHIR resources to be compatible with R5 #75

FHIR R5 was recently released. We should ensure our resources are updated to the latest and greatest release of the FHIR standard. Also, we should double check that all our attributes are being ingested correctly - i.e. compare what HAPI is storing vs. what was sent it. Regardless of what changed in R5
