Convert r4 to r5 dataset #86

As discussed, we should create a branch for the R4 resources first. Then this can be merged into Master.

Used the HAPI 6.10.5 converters to convert each resource.

I specifically utilizes org.hl7.fhir.convertors.advisors.impl.BaseAdvisor_40_50 and org.hl7.fhir.convertors.conv40_50.VersionConvertor_40_50 to convert each resource to R5.
