Include all docs in generated HTML #541

We should remove index.html from the root folder and leave only doc/index.html.

We should also decide what to do with doc/nuklear.html. Suggestions? Probably remove it altogether, right?

Looks okay. The changes to *.h are simply to the comment sections, most likely to make the html generation work.

LGTM, except the empty index.html created at the root of the project folder, why is so?

I'm tempted to merge this PR, as all seems ok.

I just have never did it on this project. Is there any preference on how a PR is merged on nuklear? @dumblob I look at you ^^

I finally chose to merge this as it has no influence on code base and having this completed doc is a plus!
