Add gazetteer creation guide #152

Added a guide to gazetteer creation. Both the text and the code is a bit rough so feel free to suggest extensive changes if need be.

I'm also wondering if once you've reviewed whether we should make a docs version for this branch and get Sam Lee's feedback on this?
