The design could be more consistent with the light and dark modes we already have. Also, I was thinking this page you created can appear after a create appointment button is clicked on the the Запись на прием page. To make the design look consistant with the light and dark modes, you need to make the background color of the card transparent. I've also noted that the design is not responsive on mobile phones. The suggestion I'm making for the design of the Запись на прием is shown in this screenshot, the screen you created can be put on the Записаться на прием button for example.
