Image upload problem #2

I found this error from the browser : indiciaData.uploadSettings is undefined.

Can you send me a link to the form showing this error, plus perhaps the login details I need to check the site configuration please?

Actually this is not only the problem for Drupal 8. If I don't use '[map]' text in Form Structure then there will be a JavaScript error appear. which stops allowing form to upload image and popping up the date.
