Added tooltips for intervention form fields
Changed intervention level to use number input that gets translated to a float between 0 and 1.
Tooltips currently just use field label.
Closes #202

@mfisher-idmod, I see that too, but it doesn't seem to be affecting anything. It may have to do with the way the tabs are loaded by Vuejs. I'll dig in.
