• [x] Country Registration Form -new person creation. check the role
• [x] Sub County Form - same as above
• [x] For sub-country form- update the question withLargest level of health facility as a dropdown with options 1-6
• [x] Ward form reads undefined when creating with theoption create new person
• [x] Community Health Area form reads undefined when creating with theoption create new person
• [x] Household form reads undefined when creating with theoption create new person
