[x] Skipto the breastfeeding question in the form if the patient is not pregnant

Currently follow up questions are asked

[x] Allow user specify if other danger sign is selected
[x] if the answer to the question Is the patient Breastfeeding? is NO, then the next question on Is the breast milk enough should not be shown

@mathewwahome , kindly confirm the form is submitted for a specific patient.

In the below example, I'm accessing patient named Akama Junior who is the household head as shown below:

But the end report submitted picks the name of the CHP, currently named Felister Kirarei

The expectation is that the report is linked toAkama Junior and should be listed here:
