Launch Form on GitHub Pages #20

It would be great if we could have a place for this form to live, why not host it on GitHub pages so that it auto-deploys and is attached to the repository?

I've set GitHub Pages up to launch from raunaq-form branch temporarily, until #16 is complete and merged, then we can move over to main branch.

I've updated the settings so now it will deploy from main branch, /docs directory.
