A couple of issues with the PDF output, at least on my machine.

The cover is not included
majordivider.gif and minordivider.gif are both scaled to the full width of the page, making them look very pixelated

Also, are we able to adjust the output formatting (I'm not familiar with MuPDF at all)? For example, I use Adobe Garamond Pro for the PDF, because that's what's used in the actual books. I've added the full list of changes I make to Calibre's default options in #6.

Hmm...you're right about the MuPDF issues. Looks like ebook-convert is a lot more configurable and may ultimately give better results. I'll do some investigation.
