adds a scroll to top action for notebooks #37

this address #11 . we added a flag and option to include a scroll to top link in the footer and an empty element at the top to recapture keyboard control. the solution for the empty tag was applied from https://css-tricks.com/need-to-scroll-to-the-top-of-the-page/

there might be some alternate solution because it seems there is ambiguity in what scroll to top should mean

these notebooks were used in recent tests. im closing before i change anything.
