When someone talks in the Shout channel and they send multiple messages quickly in there, the message sender sees all the messages in their own chat box, but everyone else only sees some of the messages because they are rate limited, causing the talker to think everyone got their messages, but some messages actually were lost. The rate limit was probably put there to prevent spam back when there was a lot of players, but I think now the population is low enough that shout spam isn't really an issue, and the loss in messages is kind of a bigger issue now (it throttles pretty aggressively and someone can just be typing normally and quickly and not trying to spam and some messages will get lost)
