What does this Pull Request do?

Adds a CONTRIBUTING.md file and updates the README.md file to point to information on how to contribute. See issue #55.

How should this be tested?

Double check the mark-down and make sure that the contents of the contributing file make sense for this project.

Additional Notes:

CONTRIBUTING.md may need to be updated as time goes on (for example, should the module be handed off to Islandora Labs.)
