Connect Patient Identity Form JS in the patient selector #159

Forms can be defined with XML/UI/JS files
All patient has the same central form.
We can force the definition of an identity form to be shown in the patient 
files AND under the patient selector with an data update following the selected 
patient in the selector. 
This way users can have a quick access to all the identity information stored 
in the patient file and in the patient database.

Partially done, JS are not connected.
