fagerstrom sub form deletes previously entered user title #348

What steps will reproduce the problem?
1. create a subform with Fagerstrom template
2. enter a name for the episode like "wants to quit bla bla bla"
3. fill in the form
4. title "wants to quit bla bla bla" is replaced by "; [[Score X]]" (X being 
the numerical result of the test)

What is the expected output?
My title with the score of the test at the end.

 What do you see instead?
"; [[Score X]]"

What version of the product are you using?
Debugg 64 bit version Linux from source pulled & compiled on April 23, 2014

On what operating system?

Debian Wheezy
