[measurement input dialog] various bugs/improvements

• radio button cannot be unchecked
• since one can input both bloodsugar and insuline use checkbox instead of radio buttons (or at least make only one of both selectable and hide the input field for the other one)
• provide the opportunity to select a time and date, because the measurement might be at another time than the actual input (-> create a time and date picker, current date and time as default. Therefore the user only needs to change the time, if the measurement occured some time before)
• refactor design (-> design guidelines)

whole measurement popup had been reconstructed on entry screen in a better looking...but still needs to get attached to the daily routine activities
