Render fields for Last Name, First Name, Middle Name, License Number, DOB, Sex, Height.

Use TextInput, SelectInput, and DateInput as needed.
Set up Last Name, First Name, and Middle Name fields.
Add Ontario Driver's License Number field.
Add Date of Birth field with date validation.
Add Sex dropdown (male, female, other options).
Add Height field with unit in cm.
