This PR updates any packages needed to remove all known vulnerabilities (i.e. "npm audit" lists 0 vulns)

Updates:

node: v12 -> v16
npm: v4 -> v8
hapi: v19 -> v20
hapi-swagger: v12 -> v14
@hapi/joi -> joi
mongoose: v5 -> v6

Breaking Changes:

Implied associations (those listed with a 'ref' property but not listed under 'associations') no longer work.
