Need to support <param name='Keyword'> #76

chmsee does not seem to support <param name='Keyword'> in chm files. Many
files, however, use it in index entries, and without it all index entries
get title 'No Title'.

Example: Python help file from
http://www.python.org/ftp/python/2.6.2/python262.chm

Opening python262.chm in chmsee yields index with all entries named 'No
Title'. Under Windows, it works fine.

ChmSee version 1.2.0-rc1 has fixed this problem.
