What steps will reproduce the problem?
1. I have download order approved.
2. I have make a template email.
3. I wil test on my shop from a order approved, on mij checkout are a button 
empty and the url don.t work.

What is the expected output? What do you see instead?

What version of Magento/extension are you using?
