Add Monorepo support #89

Idea for after #83 is released and npm 5 support is stable.

shrinkpack --lerna
  1. Not passing --lerna defaults to false.
  2. Passing --lerna instructs shrinkpack to process ./npm-shrinkwrap.json plus ./packages/*/npm-shrinkwrap.json but store the tarballs in a single cache at ./node_shrinkwrap.
