Add the ability to link external supporting documentation to a line item.

One additional table should be added to the structure of the application so 
that external supporting documentation can be linked to a line item.  This 
table should include (id, lineitem, name, link).  The item does not need to 
include a public field because it is assumed that it will inherit the line 
items privacy.

The intent of this feature is to embed external Google Docs in an iFrame so 
that supporting materials can be included as part of the application.  The 
reason for not embedding these documents directly in the application is ease 
of management.
