Add strict permission denied pages #5

While the public flag is enforced for individual pages, it is not calculated 
recursively.  For example, public receipts that belong to a private line item 
could still be accessed.  While the links would not be available, these  
pages should also be protected.

This enforcement should recursively check the permission of each line item 
back to the root to see if it is not public.
