Build header and footer from html code #355

It would be nice to have the possibility to build header and footer from html 
code and not from php-code.

We have implemented this feature and in attachment you can find a patch for 
this feature.

The patch is created on your "trunk" version revision 424.
In the attached document you find a small explanation (readme.txt) on how it 
can be used and how it works.
We have also created an example. You can find this example at your example site 
( http://localhost/dompdf//www/examples.php ) section "other" example 
"html_footer_header".

In the patch you will find comments with "@author Motrada.net". You should 
remove this comment. We have added it just to let you now, that this code block 
have been added with this patch.
It is important to say that, this feature is optional, and users would be still 
able to create headers and footers with php-code. I.e. it is backward 
compatible.
