Great background information.
Specific aims look good.
Can you categorize the associative factors (dietary, exercise, etc.)?
Some preliminary results in the write-up would be good.
Research plan needs to be way more specific.
Could you use propensity score matching to match the patients to deal with imbalance?
Experimental approach should be more specific to the data you have.

we will improve our proposal based on suggestions above, to give more preliminary information, detailed research plan, and specific experimental approach with propensity score matching for data imbalance problem.

For associative factors, we have 3 main categories: patient demographics (age, gender, ethnicity), diet (specific dietary patterns or nutritional intake), and habits (exercise frequency, tobacco use, alcohol intake).
