Section 8: There is no field on the form to data entry the phone number #14

Section 8: There is no field to enter phone number of reporting person. Please add a box between the Reporting person's name and date. There is a label on the PDF for this to go, but nothing is printed on the PDF, since nothing is entered in the form. The field in REDCap for this is report_phone. See attached screen shots. Section 8 contact number no field for on form and not printed out on the PDF.docx
