Had an explosion (in the form of a creeper in a tree) kill me. Had a grave/body type mod so all my items stayed. But the explosion 'popped' my broom, and five minutes later it despawned. It didn't help that it was probably in a tree.
The whistle didn't work, but I realize my later that the reason was that the broom 'popped' back to item form. I tested it in singleplayer to find the same.
