Mixed dose types #3

Currently, there seems to be an issue where the correct amount of dosages is only counted if the first of a mixed set of doses is an infusion dose. If the first dose is a bolus dose, then all infusion doses seem to be skipped

An issue has been raised for rxode and will await further instructions from them

The issue has now probably been fixed. Awaiting merge of fix in rxode2

The issue has been fixed with the implementation of merge in rxode2. However, the dose count does still seem to be wrongfully made.
